Who should use the forms on this page? 

The forms found on the bottom of this page are for the use of Trials PAC members ONLY. Members should use these forms to submit updates and firm profiles for inclusion in PAC’s marketing materials.

When will I see my changes reflected in the materials? 

Trials PAC materials are updated twice a year. Once, for use at the Spring Meeting every February and once for use at the Annual Meeting. You may submit a form at any time, but unless approved by one of the Trials PAC’s Co-Chairs, Sandy L. Morris or Leticia “Tia” Stevens, the materials will only be updated in preparation for these meetings and not outside of these scheduled updates. Special exceptions can be made, so please contact the Trials PAC Co-Chairs if you feel your firm’s info needs to be updated immediately. 

Form submission deadline for 2026 Annual Meeting.

Only changes from form submissions that are received before the deadline are guaranteed for inclusion in the print materials uploaded in time for the next NAMWOLF event. The deadline for submissions for the 2026 Annual Meeting is TBD.

Trials PAC New Member General Information Form

  • (please type the name as it should appear in the marketing materials)
  • Here you can give us your firm's web address for the home page, or any page on your website that you'd like the TRIALS PAC marketing materials to direct to for your firm.

  • Procedural Capabilities, Key Specialty Areas and Industries

    In this section, please let us know which Procedural Capabilities your firm offers, if any (Arbitration, Appeals, Class Actions). As well as selecting up to six Specialty Areas and/or Industries that reflect your firm's trial practice. For the purposes of the map directory chart, our corporate members have requested that our firms narrow down/simplify their selections. For this reason we limit your selection to six. Please utilize your one-page firm profile to highlight all your firm has to offer. The profile sheets can be found after the map directory chart section of the PAC's marketing materials. The profile pages are also hyperlinked via your firm's logo in the map directory chart, so that in-house counsel can find your profiles two ways.
  • Please check as many of the following capabilities that apply to your firm. If none, please leave "none" checked as this is a "required" field.

  • Firm Profile Sheet & Logo Upload

    Please upload a one-page firm profile sheet here. Your profile should highlight your firm's Trials experience as well as provide the contact information of your firm's Trial PAC contact or contacts. If you do not have a profile to upload at this time please use the RETURNING MEMBER FORM to do so before the deadline at the top of this page (you will only need to fill out the firm name portion of the form, upload your profile at the bottom and hit submit—no other fields are required). Any form or profile submissions received after the deadline are not guaranteed for inclusion in the PAC materials being uploaded prior to the next conference.
  • Dimensions: 8.5" x 11" (NO BLEEDS)
    Max. file size: 100 MB.
  • Please upload a high resolution version of your firm's logo. JPG or PNG only please. If your firm has multiple versions of its logo, please upload the version that is closest to a 2 x 4 horizontal rectangle shape, as this shape will display best in the materials. Any shape will work, however.
