Who should use the forms on this page?

The forms found on the bottom of this page are for the use of Financial Services Litigation PAC (“FSL PAC”) members ONLY. Members should use these forms to submit updates and firm profiles for inclusion in PAC’s marketing materials. To review the last produced versions of these materials, click here:  Last-produced PAC Directory 

If your firm is already listed in the last produced marketing materials (linked above) and the information is correct, no action is needed.

One-Page Firm Profiles

Members are encouraged to provide a One-Page Firm Profile for inclusion in the Directory. See examples in the back of the PAC Directory linked above.

Can I email my updates/firm profile instead of using a form?

To ensure that your information is included in the PAC’s marketing materials accurately, please use the forms and do not send any additions, changes or updates by email. If for some reason a form does not function properly you may email your updates and firm profiles to matt@rothstokes.com.

When will I see my changes reflected in the materials?

FSL PAC materials are updated twice a year. Once, for use at the Driving Diversity & Leadership Conference (formerly known as the Business Meeting) every February and once for use at the Annual Meeting & Law Firm Expo every September. You may submit a form at any time, but unless approved by one of the FSL PAC’s Co-Chairs, the materials will only be updated in preparation for the meetings. Special exceptions can be made, so please contact the FSL PAC Co-Chairs if you feel your firm’s info needs to be updated immediately.

Deadline for inclusion

Only changes from form submissions that are received before the deadline are guaranteed for inclusion in the print materials disbursed at the next NAMWOLF event. The deadline for submissions for the 2024 DDLC is January 31, 2024.

  • If you have more than one firm location please list this way: City, State abbr.; City, State abbr.; City, State abbr.
  • Please select all that apply. To select multiple states hold "Ctrl" while selecting.

  • Firm Profile Sheet Upload

    Please upload a one-page firm profile sheet here. Your profile should highlight your firm's FSL experience as well as provide the contact information of your firm's FSL PAC contact or contacts. If you do not have a profile to upload at this time please use the RETURNING MEMBER FORM to do so before 06/30/21 (you will only need to fill out the firm name portion of the form, upload your profile at the bottom and hit submit—no other fields are required). Any form or profile submissions received after 06/30/21 is not guaranteed for inclusion in the PAC materials being distributed at the upcoming Annual Meeting.
  • Max. file size: 100 MB.
  • After hitting the "Submit" button, please allow a few moments for your form to go through. You will be redirected to a confirmation page once it has finished. If you are uploading a profile sheet this process will take longer. If your form will not go through with a profile sheet uploaded, please submit your form without including the profile sheet and then email your profile to matt@rothstokes.com. Thank you
